I was approached by a team out of Henry Ford Hospital's Innovation Institute to help tackle the problem of high readmission rates.
Our goal was to bridge hospitals, primary care physicians, pharmacies, & health care providers in an effort to keep patients on the right track regarding medication regimens and follow-up appointments.
Through increased communication and accountability in post-op care, we aimed to reduce hospital re-admittance rates in the 30-day window following release.
Working with Henry Ford doctors and developers, we designed a solution in a combination of mobile and desktop applications focused on:
Events: Keeping patients informed of follow-up appointments and hospital staff alerted upon missed appointments.
Medication: Full-fledged medication tracking functionality with patient-specific dosage instruction.
Activity: A record of patient activity and 30-day progress,
Patient screen: One screen for healthcare workers to view and manage all patient activity, as well as contact information for the patient's entire healthcare loop. (Primary care physicians, healthcare providers, and pharmacies.)
Medication tracking and follow-up appointment schedules with alerts to keep patients on track.
Alerts hospital staff to struggling patients, manages patients' iOS app data, and facilitates communication between the entire entire healthcare loop.
